Absentee Notification -This form is to be used by all GMCCC Leadership in the event that they will not be able to attend any services, functions or events.
Program Proposal - This form is to be used when any program, event or special service is to be conducted under GMCCC.
Budget Request - This form is to be used when any funds are needed or being requested for ministry work at GMCCC.
Contact Report Form - This form is to be used by GMCCC Membership representatives only. It is used for reporting purposes; when any type of contact has been made with another GMCCC member or candidate. This form requires a password. Please see the Deacon/Deaconess Chairpersons or the Christian Education Department for access.
Ministry Evaluation Form - This form is to be used by Overseers and Ministry Heads. It is used to evaluate the overall performance of a GMCCC Ministry and its Leader.
Online Payment Form - This form allows you to make payments using a credit or debit card. Donations, tithes, tuition, fees, materials, offerings, and any other payment to GMCCC or WTM can be made utilizing this secure form.
WTM Store Order Form -This form allows you to order and purchase WTM products such as books, audio and video series, music, T-shirts and more.
Tithing/Pledge Form - This form allows you to complete and submit your GMCCC Tithing and FaithWalk2000 pledges. It is confidential and will be sent directly to our finance office.
